We are looking for two experienced and committed individuals to join us as Senior Practitioners and help in the delivery of an excellent service within two different teams working within the Lambeth Living Well Network; the Living Well Network (LWN) Hub and the Alliance Rehabilitation Team (ART).


The LWN Hub:


The LWN Hub is the gateway service for all mental health referrals in Lambeth, it is a multi-disciplinary team consisting of community psychiatric nurses, social workers, occupational therapists, peer support, psychiatric support and support workers from Thames Reach.


As Community Coordinator (Senior Practitioner) you will work closely with the locality coordinators, clinical staff, and the Hub Coordinator to oversee the caseload of service users presenting to the Hub, ensuring that all people referred and presenting are responded to quickly and appropriately. You will line manage support staff within the locality team.


The Alliance Rehabilitation Team (ART):


The ART supports people with people with long term enduring mental health issues who live in residential/nursing homes, are on the hospital rehabilitation wards or may be on a trajectory for needing those services. It is a multi-disciplinary team consisting of community psychiatric nurses, social workers, occupational therapists, psychiatrics and support workers from Thames Reach.


As Senior Practitioner in the ART you will work as part of the ART management structure to support the team to deliver an effective service, providing an excellent standard of support to its service users. You will line manage Thames Reach support staff within the team.


To join us as Senior Practitioner you will need:


·         Ability to work quickly and have strong networking and relationship building skills.

·         Experience of and adapting to processes in periods of change.

·         A track record of working in support services for a range of vulnerable people

·         An understanding of the needs and aspirations of people with mental health needs

·         The ability to set standards and inspire your team to enable every service user to receive the highest standard of support from us. 


You will work in Lambeth and will be based in our Waterloo, Stockwell or Brixton offices and will travel across the borough in order to meet the needs of the service.
